FLOWCHART FOR UPCAT APPLICATIONS AND FEEDBACK/SUGGESTIONS

FOR ALL APPLICANTS:

Completely accomplished AND signed UPCAT Forms 1 & 2
4 recent 2x2 ID photos
If the applicant is a Transferee from another HS (e.g., for summer courses):

SUBMIT a certified photocopy of the Permanent Secondary School (F137) from the other school & attach to UPCAT Form 2;
If the applicant is a Member of and Indigenous Group:

SUBMIT a CMG certificate from the National Commission of Indigenous Peoples;
If the applicant is already a HS graduate:

SUBMIT HS Diploma
